 We Provide Door-To-Door Transportation 

Nexus Social Adult day care provides services to those who have Medicaid or qualify for Medicaid as well as private pay. At our center you will be able to meet new friends, socialize, enjoy our breakfast, snacks and hot lunch together, do exercise, play games such as air hockey, table tennis, bingo and many more.

We have group discussions and we do our best to keep our participants up to date with the current events. Transportation is provided . Please stop by to check our Social Adult Day Care Center which is located in the heart of Floral Park, New York.

Our Center will provide senior care services from 9AM - 2PM Mon-Wed-Fri. During that time we will offer nutritious meals, snacks, along with age-appropriate structured weekly lesson plans and age-appropriate activities.
